These fields collect written input from clients.

  • Single Text Line: A single-line input for short answers: names, reference numbers, job titles.
  • Text Area: A multi-line input for longer written answers: descriptions, background summaries, open-ended responses.
  • Text Description: A read-only text block set by you, not the client. Use it for instructions, compliance notices, or context that needs to appear on the PDF download.

Contact & Identity Fields

These fields collect structured personal or contact information.

  • Name: Collects first and last name in separate inputs.
  • Address: Collects a full address with separate inputs for street, suburb, state, postcode, and country.
  • Phone: Collects a phone number in a validated, consistent format.
  • Link: Collects a URL, such as a website, LinkedIn profile, or portfolio.

Selection Fields

These fields let clients choose from a predefined set of options.

  • Multi-Choice: Clients can select one or more options from a list. Best when multiple answers can apply at once.
  • Single Select: Clients choose exactly one option from a dropdown list. Best when only one answer is valid.
