What this field does: The Name field collects a structured name from clients, with separate inputs for first name, last name, and optionally middle name and title/prefix.
How to add a Name field
- Open your request in the Request Builder.
- Select “+ Add a field” and click Name to add it to the current section.
- Enter a Field Name.
- Click the field on the field cog to open its settings.
- Choose which name components to include (e.g. first name + last name only, or include title and middle name).
Available field settings
- Mandatory: requires the client to complete this field.
- Components: choose which parts of the name to collect: Title, First Name, Middle Name, Last Name.
What clients see
Clients see separate labelled inputs for each name component you’ve selected.
